This is a Gopher team meeting for parents and players to kick off the season. Here is the agenda:
1. Randy opens the meeting with a review of the agenda
2. Introductions – Lance introduces coaches
3. Assistant Coaches each share a little bit about themselves
4. Lance shares a little bit about himself, plans for the season, expectations, etc.
5. Player/Parent introduction
6. Randy covers administrative stuff
a. Paperwork
b. Practice schedule
c. Volunteering/DIBS
d. Team website
e. Communications
7. Questions?
8. Handout team uniforms
See below for documents to complete and bring to the meeting. It will take approx. 15-20 minutes to fill out all the forms. Forms can be found on the OMGHA website.
Each player's parents must print, fill out and sign the following forms:
1. Code of Conduct Form (Read this to your player - they have to sign it too!!)
2. Consent to Treat Form
3. Waiver of Liability Form
4. Volunteer Buyout Form (Fill this out whether you've bought out or are participating).
5. Bring a checkbook/blank check for the Volunteer Buyout program. $500 deposit per skater if volunteering (this will be ripped up at end of season once your hours are complete).
6. Bring a checkbook/blank check for the team fund - we'll discuss this at the parent meeting.
Each Coach must:
1. Bring a copy of your CEP card (front & back), or bring your CEP card, to the meeting tomorrow.
2. Fill out a Background Check Form. If you haven't already filled out a Background Check Form when you signed up to coach, you need to fill one out and bring to the parent meeting. This needs to be turned in and processed before you can get on the ice.
3. Go to the USA Hockey website http://www.usahockeyregistration.com
and register yourself (this is insurance). OMGHA will reimburse you for this.
